Job Summary

The post holder will visit patients directly in their own homes to deliver hospital-quality physiotherapy care in a setting that is familiar, dignified and comfortable for the patient and their family. This includes assessing mobility, function and pain, designing and delivering individualised rehabilitation programmes, and supporting patients recovering from surgery, stroke, injury or living with long-term physical conditions.

Each physiotherapist is expected to visit an agreed caseload of patients per day across assigned locations, recording all assessments, treatment sessions and progress directly on the Jourwell app so that the patient's care team, family and attending doctor can track progress in real time.

The post holder will liaise directly with each patient's doctor and wider care team to ensure that rehabilitation delivered at home remains medically supervised and properly integrated with the patient's overall treatment plan, rather than informal or unsupervised.

They will contribute to Jourwell's clinical audit and quality assurance processes, working alongside the Clinical Manager to maintain high standards of care and to support the continuous improvement of the Hospital-at-Home service model.

Key Responsibilities

To be responsible for the assessment of mobility, function and rehabilitation needs, and the delivery, implementation and evaluation of individual physiotherapy care plans, using a problem-solving approach and evidence-based practice.
To visit an agreed caseload of patients per day in their homes, providing safe, effective, person-centred physiotherapy care in accordance with Jourwell protocols.
To design and deliver individualised rehabilitation and exercise programmes for patients recovering from surgery, stroke, orthopaedic injury, neurological conditions, or managing chronic pain and age-related mobility decline.
To perform manual therapy, therapeutic exercise, gait and balance training, and to advise on the safe and correct use of mobility aids and equipment.
To accurately record assessments, treatment sessions, progress and outcome measures using the Jourwell app at the point of care.
To develop and maintain effective working relationships with each patient's doctor and wider care team, ensuring seamless communication, shared decision-making and continuity of medically supervised care.
To recognise signs of clinical deterioration, complications, or safety risk during rehabilitation promptly and escalate concerns in line with agreed protocols, including coordinating safe and timely transfer to hospital where necessary.
To support families caring for elderly or chronically ill relatives at home, respecting the cultural preference in many Nigerian households for home-based care over residential/nursing homes, and offering a dignified, hospital-quality alternative.
To receive and act on referrals for new patients, carrying out initial assessments and agreeing a home-based rehabilitation plan in partnership with the patient, family and referring doctor.
To demonstrate professional competence in advising and supporting patients and family caregivers, including education on home exercise programmes, fall prevention, safe moving and handling, and injury/re-injury prevention.
To maintain accurate, timely and legally compliant records on the Jourwell app and any associated systems, in accordance with the Nigeria Data Protection Act 2023 (NDPA) and Jourwell's information governance policies.
To act as a role model for junior physiotherapists, student therapists and community health workers, representing Jourwell's HEART values at all times.
To participate in Jourwell's clinical audit, quality assurance and continuous improvement processes alongside the Clinical Manager.

The Main Demands of the Position Are

Manual moving and handling of patients and equipment, including supporting patient transfers, assisting with exercises, and carrying equipment bags (mobility aids, therapy bands, portable treatment tools) between patients' homes.
Travelling independently, on foot or by vehicle/rideshare, to multiple patient locations across Lagos in a single day, including navigating traffic and varied home environments.
Working alone in a patient's home for the duration of each visit, without on-site colleague support, while maintaining sound clinical judgement, particularly during physically assisted exercises.
Coping calmly with working under pressure and time constraints, while maintaining concentration for clinical decision-making and patient safety during physically demanding sessions.
Communicating professionally and sensitively with anxious, frustrated or distressed patients and family members, including when supporting patients adjusting to disability, discussing slow or limited progress, cognitive impairment, or language/cultural differences.

To ensure a good public image of Jourwell Homecare at all times, and to act as a role model for junior staff and students, representing Jourwell's values and the standards of the Medical Rehabilitation Therapists (Registration) Board of Nigeria (MRTB).
To report all incidents, near-misses and complaints in accordance with Jourwell policy.
To be aware of relevant health and safety legislation applicable in Nigeria and to work in a manner that protects the safety of patients, families and colleagues.
To uphold the principles of the MRTB Code of Professional Conduct at all times.
To hold and maintain a current, valid MRTB practising licence for the duration of employment.

General

To comply at all times with Jourwell Homecare's policies, especially those governing health and safety, infection prevention and control, and to report promptly any equipment defects or safety concerns to the Clinical Manager.

This job description reflects the present requirements of the post. As Jourwell's services develop, the job description will be reviewed in consultation with the post holder.

You are required to disclose any additional clinical work you undertake or plan to undertake for another employer.

Probationary Period

This post is subject to the successful completion of a probationary period of three (3) months, within which the notice period for both the post holder and Jourwell Homecare is two weeks.

Standards

Staff are responsible for complying with the relevant clinical and conduct standards set by the Clinical Manager. A breach of such standards may lead to disciplinary action, which will be investigated fairly with appropriate steps taken to prevent recurrence.

Information Governance

You are required to respect the confidentiality of all patients, families and staff, and must not share any information obtained in the course of your duties. Any non-compliance must be reported through Jourwell's Incident Reporting process.

All staff must comply with the Nigeria Data Protection Act 2023 (NDPA), Jourwell's IT Security and Information Governance Policies, and Best Practice Guidelines.
