Job Summary

The post holder will visit patients directly in their own homes to deliver hospital-quality nursing care in a setting that is familiar, dignified and comfortable for the patient and their family. This includes monitoring vital signs, managing medication, performing clinical procedures such as blood draws and wound care, and identifying early signs of deterioration.

Each nurse is expected to visit 3–4 patients per day across assigned locations, recording all observations directly on the Jourwell app so that the patient's care team, family, and attending doctor can track progress in real time.

The post holder will liaise directly with each patient's existing doctor or hospital to ensure that care delivered at home remains medically supervised and properly connected to the wider healthcare system, rather than informal or unsupervised.

They will contribute to Jourwell's clinical audit and quality assurance processes, working alongside the Clinical Manager to maintain high standards of care and to support the continuous improvement of the Hospital-at-Home service model.

Key Responsibilities

1. To be responsible for the assessment of care needs, and the delivery, implementation and evaluation of individual care plans, using a problem-solving approach and evidence-based practice.
2. To visit an agreed caseload of 3–4 patients per day in their homes, providing safe, effective, person-centred nursing care in accordance with Jourwell protocols.
3. To accurately measure and record vital signs (blood pressure, pulse, temperature, oxygen saturation, blood glucose and other observations as required) using the Jourwell app at the point of care.
4. To perform clinical procedures including venepuncture, cannulation, intravenous therapy (including IV antibiotics where prescribed), wound care and medication administration and management.
5. To develop and maintain effective working relationships with each patient's existing doctor, hospital or clinic, ensuring seamless communication, shared decision-making and continuity of medically supervised care.
6. To recognise signs of clinical deterioration promptly and escalate concerns in line with agreed protocols, including coordinating safe and timely transfer to hospital where necessary.
7. To support families caring for elderly or chronically ill relatives at home, respecting the cultural preference in many Nigerian households for home-based care over residential/nursing homes, and offering a dignified, hospital-quality alternative.
8. To receive and act on referrals for new patients, carrying out initial assessments and agreeing a home-care plan in partnership with the patient, family and referring doctor.
9. To demonstrate professional competence in advising and supporting patients and family caregivers, including education on medication compliance, chronic disease management and healthy living.
10. To maintain accurate, timely and legally compliant records on the Jourwell app and any associated systems, in accordance with the Nigeria Data Protection Act 2023 (NDPA) and Jourwell's information governance policies.
11. To act as a role model for junior nurses, student nurses and community health workers, representing Jourwell's HEART values at all times.
12. To participate in Jourwell's clinical audit, quality assurance and continuous improvement processes alongside the Clinical Manager.

The Main Demands of the Position Are

• Manual moving and handling of patients and equipment, including bending to change dressings, setting up infusions and carrying equipment bags (vitals monitors, phlebotomy kits, medication) between patients' homes.
• Travelling independently, on foot or by vehicle/rideshare, to multiple patient locations across Lagos in a single day, including navigating traffic and varied home environments.
• Working alone in a patient's home for the duration of each visit, without on-site colleague support, while maintaining sound clinical judgement.
• Coping calmly with working under pressure and time constraints, while maintaining concentration for clinical decision-making.
• Communicating professionally and sensitively with anxious or distressed patients and family members, including when discussing difficult diagnoses, cognitive impairment, or language/cultural differences.
